TEST SMART: HOW TO PASS NCLEX
Though I’m extremely excited for this weekend’s trip to Davis…
I’m simultaneously worried that I won’t have enough time to study. Am I tripping out or something? I have so much to cover, so much to review, that my brain is about to explode. Or for the lack of information my brain is not sensing, implode. I have critical care to cover, neuro to re-study, pregnancy nursing care for pre- intra- and post- partum, pregnancy alterations and the care, cancers, pediatric care, pediatric developmental guidlines, pediatric care for all their alterations, oh gosh.
I’m dead, and I haven’t cracked open a book. I see my “Musicophilia” book that I bought last quarter sitting infront of me. Ready for a read during my time of leisure. I want to read it. But my nursing bible is calling to me.
Been up since 6am and all I’ve been doing is tumblring. Must stop. Darnit.
Last Med-Surg Clinicals with this awesome group for the nursing program. Can’t believe I’m going to be a preceptee in the fall. Nursing school. Holy crap.

Stressful last day at clinicals. Need to get away from the world for a second.
It is the penultimate clinical of the quarter!!!
And tomorrow is my final day as a Med-Surg Nursing student. Next time I see the Hospital floor, I’ll be a preceptee!! It’s scary.
But anyways, it is the second to last time I have to wake up at 0400 am in the morning to get ready and commute to the hospital. Never thought I’d make it this far. Crazy….

Nothing exciting has hit me except for the fact that my stress level is down…
and that my BP is WNL—within normal limits, thank God. This weekened was a breath of fresh air. I got to be with friends, I got to roam around San Francisco, I got to sit down in coffee shops (aka Starbucks) and pretend to study but actually people-watch (HAHAHA), I got to hang out with friends, I got to run away from the nursing world, I got to run away from school for a good four days.
And now I’m back. Looking at my notes, trying to intiate a study pattern. But the truth is, I have no idea where to start anymore. I should be worrying. I am worrying. If I fail Med-Surg, my life is over. Hahaha. But I need to study. I hate how my 2/3 of my nursing professors aren’t even teaching. I’m used to being lectured, take notes, and think critically, but they are not even giving us the pertinent information for the test. I feel like being a 5th quarter (out of 6) nursing student, that I should know how to process the information, and answer any kind of nursing type of question using my knowledge, my nursing process, and my critical thinking. I can’t. I answer these questions and always get them wrong!
Something is wrong with me. I got a near F on the last med-surg test. I can’t do it again on this test. God forbid that to happen. But if it does, it looks like I get more practice in the hospital setting. I’m surviving with an 81% test average, but I can’t get lower than a 70% or else I’m out of the program. Goodness… I need to do well. I guess I better get off tumblr. I should deactivate this account for two weeks?
Sound like a plan? Yes.
I. Prioritizing - Means ranking the client’s problems in order of importance depending on:
- Issue of the question
- Clinical setting
- Client’s condition
- Needs/problems that require immediate attention
A. Priority
- High priority - life threatening conditions if untreated would result in harm/injury
- Intermediate priority
- Low priority
B. Guides for prioritizing
- Keywords or key phrases
- Maslow’s Heirarchy of Needs Theory
- Physiological needs are the first priority (Airway, Breathing, Circulation)
- Pain
- Safety
3. Steps of the Nursing Process
- Assessment vs. Implementation (Assess first)
- Expected outcome
C. Other priorities
- Most acute/least stable patient
- Complication of the disease condition (not an expected outcome)
II. Delegation and Assignment-making
A. Principles and Guidelines
- Ensure client safety
- Focus on what the question is asking for
- Determine which activity can be delegated safely and legally.
- Match the activity on the basis of the nurse practice act.
- Provide adequate supervision (supervise RNs who are new grads.)
B. Who can do that?
- Unlicensed personnel - Noninvasive tasks and basic client care activities that include the following:
- Ambulation
- Bathing
- Client support
- Grooming
- Hygiene measures
- Positioning
- Range-of-motion exercises
- Skin care
- Some specimen collection, such as urine or stool.
2. Licensed Practical or Vocation Nurse - Certain invasive tasks and client care activities that include the following:
- Administering oral medications
- Administering intramuscular and subcutaneous injections
- Changing dressings
- Irrigate wounds
- Monitoring an intravenous flow rate
- Suctioning
- Urinary catheterization
3. Registered Nurse - Some of the tasks and client care activities that only the registered nurse can perform are as follows:
- Administering intravenous medications
- Leading others and managing the client care environment
- Teaching
- Using the nursing process: assessment, analyzing data, planning client care, implementing care and evaluating care
III. Test-taking Techinique
A. Identify the parts of a question
- Case situation
- Question stem
B. Read the question carefully. Look for keywords or phrases in the case situation and stem the question
Examples:
- What is an early sign of shock?
- What is the initial nursing action? Indicates that options are correct and you have to prioritize.
- Which statement by the client indicates understanding of the instruction? Indicates a true response questions
- Which statement by the client indicates the need for additional teaching? Indicates a false response question
C. Identify the issue. (What is the problem asking?)
D. Use the process of elimination. Involves reading each question and removing options that are incorrect and do not address the issue of the question.
E. Avoid asking yourself “what if?” or “reading into the question”. Means that you are considering issues beyond information presented in the question. Moves you off track with regards to what the question is asking.
F. Additional tips and strategies
- Eliminate options that contain absolute words e.g. all, always, never, none, only
- Focus on nursing rather than medical interventions.
- Ensure that all parts of an option are correct.
- Look for an umbrella option. (more comprehensive answer)
- Visualize the information (arrange in sequence/order)
- Look for the option that relates to the question.
- Don’t expect the test to end after 75 questions!

OH and my vegan clinical instructor…
made a joke about an animal today that really surprised me. It surprised me how a vegan would joke about something as terrible as what he said. I’m not going to say it because it is just too morose.
But me and my vegan classmate were dying laughing. Oh vegans. You’re a hoot. ;)
So a Filipino Grandma gave me her digits at clinical today…
even when I told her that is illegal. Awkwardest moment of my nursing life. She called me in every 15 minutes just to speak tagalog to her… and I don’t even SPEAK tagalog made it even more irksome. But I delt with it. Just had two patients, charted on four of my nurses patients, catered to most of their needs.
I still do not know how I’ll be able to handle three, or even four, patients by being the primary nurse. Shudders. Wish me luck on my preceptorship next fall. I hate this summer break before my last quarter. It is utterly inconvenient.
And for the record, even though she wrote down her number, I threw it away in the confidentiality bins and said “I’m sorry ‘nang!” She was hurt, but I explained to her. -___- awkwardest to the max.
Ran into my music professor from my ex-music major life…
and I’m really sad that I don’t have my basson chops up because they’re playing Lord of the Rings, Spiderman, and Pirates of the Carribean for their pops concert!!! MANNNNN. I hate this. But I have nursing on my mind. T_T
But I was talking to her and I really want to go back and get my Associates in Music up here at PUC. Just because I love the program, know the people, and have all and MOST of the pre-requisites to get my music degree. T_T
Options options. Get a job up here? I don’t know. But I loved my music professor. Oh I miss her. She was so helpful and encouraging. Ahh. Goodness. It would be awesome to get a Bachelors in Voice Performance and a Masters in Conducting. :(

OB class is hilarious…
I did not know that there were so many modes of contraceptives or “situational” contraceptives. These jokes during nursing class are hilarious. Currently dying. Can’t wait to educate my patients about this subject. Bahahahahahaha
Appropriate! Basically my baby today, but my baby was not under phototherapy like this kiddo. This baby most likely had jaundice, but it seems to have subsided due to the phototherapy.

Had an amazing day with the babies today…!
:))) I LOVE BABIES. LOVE THEM! LOVE THEM LOVE THEM LOVE THEM. Period. I love babies. No matter how much they cry and poop and pee. I love them! Intensive Care Unit with my babies, tube feedings, position changes, diaper changes, vital signs, assessments, medication administration.
I love babies. I was super scared to handle my baby though because she was only 21 days old, born at 28 weeks—for you non-nursing/medical folk that follow me, Term babies are from 38-40 weeks. So she was and still is premature. Only 3 lbs :(. But I tried my best to keep her comfortable, and keep her vitals stable.
She was super cute. I want a baby.
But now I just finished working out. Eating, and I’m going to tackle my OB project -__-. Ughh… I have so much sto study for that it is not funny. Not funny at all. I hate the theory part of nursing school. Hate it.
On the other hand, I found my spiritual/nursing-student/anxiety-24-7 twin. Well she is a twin in my nursing situation. Longer story later. Time to get cracking.
