EMERGENCY CARDIAC DRUGS (Adult Dosage/Administration, Use)
1. Adenosine
- 6mg rapid IVP, then 12mg IV q 1-2min X 2 prn
- half life <10sec; give at closest IV port, follow with IV flush
- -symptomatic SVT
2. Alteplase (tPA)
- 15mg IV bolus followed by infusion 0.75mg/kg over next 20minutes followed by infusion 0.5mg/kg over next 60minutes
- -fibrinolytic; treatment of acute developing myocardial infarction
3. Amiodarone
- rapid infusion 150-300mg IV over 10min
- follow with 1mg/min infusion
- -persistent or recurrent VF or pulseless VT
4. Atropine
- 0.5-1.0mg IV q3-5min prn max 0.03-0.04mg/kg
- can be given through ET tube 2-2.5times normal IV dose
- -symptomatic bradycardia, asystole
5. Dobutamine
- continuous IV infusion; usual does is 2-20mcg/kg/min IV, based on patient response
- -short-term management of cardiac decompensation resulting from depressed contractility
6. Dopamine
- continuous IV infusion: dose range: 5-20mcg/kg/min; begin infusion at 5mcg/kg/min; increase infusion rate according to BP and other clinical responses
- -hemodynamically significant hypotension in the absence of hypovolemia
7. Epinephrine
- 0.1-1.0mg IV; repeat q5min prn
- can also be given through ET tube 2-2.5 times normal IV dose
- -cardiac arrest, anaphylaxis, bradycardia
8. Isoproterenol
- IV 0.02-0.06, then 0.01-0.2mcg/min
- -heart block, ventricular dysrhythmias
9. Lidocaine
- 1.5mg/kg to total of 3mg/kg IV; follow by continuous infusion 1-4mg/min
- can be given through ET tube 2-2.5 times normal IV dose
- -VT, VF, PVCs
10. Magnesium
- 1-2g IVP
- -torsades de pointes, or when dysrhythmia caused by hypomagnesium
11. Nitroglycerin
- SL 0.4mg
- IV infusion: initiate at 5mcg/min, increase every 3-5min until pain free, BP >90, or maximum dosage 20mcg/min
- ischemic chest pain; improves coronary perfusion
12. Norepinephrine
- IV infusionL initiate infusion at 2mcg/min and titrate to desired BP
- usual range= 2-12mcg/min
- -short-term use for hypotension or shock
13. Procainamide
- 20-30mg/min, max= 17mg/kg or 500 mg
- -recurrent VF, VT, PVCs
14. Propranolol HCl
- IVP: 0.5-3mg
- give slowly 1mg/min; may repeat dose after 2min
- -control of hypertension and suppression of rapid-rate cardica dysrhythmias
15. Reteplase
- 10units IV bolus over 2min; after 10min, administer 2nd 10units IV bolus over 2min
- -fibrinolytic; treatment of acute developing infarction
16. Vasopressin
- dilute with D5w
- cardiac arrest: 40units IV x 1
- other: 0.5milliunits/kg/hr
- max: 10milliunits/kg/hr
- -alternative to epinephrine for VF or pulseless VT; short-term use for hypotension or shock
